Clinical study on the effect of abnormal anatomical structure of nasal cavity and paranasal sinus on the pathogenesis of chronic rhinosinusitis

Abstract：Objective To clarify the effects of different abnormal nasal sinuses anatomic structure in the development of chronic rhinosinusitis.Methods Retrospective cohort study was conducted. One hundred patients of chronic rhinosinusitis (CRS group) and 80 patients of non-CRS (non-CRS group), which were from the Department of Otorhinolaryngology of First People's Hospital of Bengbu and the First Affiliated Hospital of University of Science and Technology of China in January 2014 to June 2017,were enrolled in The retrospective cohort study. To evaluate the differences of the nasal anatomy abnormality between the CRS group and the control group by endoscopic nasal and sinus CT.Results The incidence of deviation of middle nasal septum, hypertrophy of inferior turbinate, hypertrophy of bubbly middle turbinate and uncinate process in CRS group were 56% (56/100), 23% (23/100), 30% (30/100) and 21%(21/100), respectively, which were higher than those in non-CRS group (χ2=14.701, 4.198, 5.590, 3.979, all P values＜0.05), in which incidences were 27.5%(22/80), 11.25%(9/80), 15.0%(12/80) and 10.0% (8/80). The incidence of middle turbinate deflection was 16% (16/100) in CRS group and was 7.5% (6/80) in non-CRS group. There was no significant difference of uncinate process gasification between the two groups (P＞0.05), in which the incidences of uncinate process gasification were 3% (3/100, CRS) and 1.25% (1/80, non-CRS), respectively.Conclusions Nasal septum deviation, inferior turbinate hypertrophy and agger nasi cell are three most common kinds of abnormal anatomical structure in CRS patients, which play important roles in the development of CRS. Therefore, it is important to distinguish anatomy variations in the endoscopic treatment of CRS.
