Comparison of three measurement methods of femoral anteversion angle

Abstract:Objective This study aims to evaluate the correlation and differences of three methods for measurement of femoral anteversion angles. Methods Ninety-one left femurs were selected for computed tomography (CT) data analysis, and 3D surface models were generated using computer software. The models were imported into computer-aided software to measure the femoral anteversion angles by using traditional anatomical methods, CT-based Reikerås method, and Murphy method. The following indicators were observed: (1) inter- and intra-observer consistency of the three measurement methods; (2) correlation and differences among CT-based Reikerås method, Murphy method, and the traditional anatomical measurement method as well as the reference values of CT-based Reikerås method and Murphy method. Results The femoral anteversion angle values were 14.8°±5.0° in the traditional anatomical method, 13.3°±7.2° in the CT-based Reikerås method, and 21.8°±7.3° in the Murphy method were. Good inter- and intra-observer consistency was found among the three measurement methods (the interclass correlation coefficient ranged from 0.960 to 0.987). The measurement results obtained by the traditional anatomical method were strongly correlated with those obtained by the CT-based Reikerås method and the Murphy method (r=0.905, 0.942; all P values <0.001), and the differences were statistically significant (t=4.10, -21.52; all P values <0.001). The reference values for measuring the femoral anteversion angle by using the Reikerås method and the Murphy method were 13.7°-15.8° and 20.3°-23.3°, respectively. Conclusion The CT-based Reikerås method and the Murphy method for measuring the femoral anteversion angle are strongly correlated with the traditional anatomical method, but their measurement and reference values differ.
