Abstract:Objective To discuss the efficacy in using nanocrystallinecollagen basal allogeneic bone brace combined with allograft for the treatment of adults' early necrosis of femoral head.Methods Clinical data of 104 patients (122 hips) with Steinberg stage Ⅰ-Ⅲ adult's necrosis of the femoral head in Department of Orthopedics, Union Hospital, Tongji Medical College, Huazhong University of Science and Technology from March 2012 to November 2014 were retrospectively analyzed, there were 85 males and 19 females, mean age was 31.2 years old (21-47), Steinberg stage: stageⅠ 35 hips, stageⅡ 72 hips, stage Ⅲ 15 hips. They were treated with self-designnanocrystalline collagen basal allogeneic bone brace combined with allograft. All patients were followed up with osteonecrosis progress and collapse of femoral head. Harris hip score changes and VAS scores before and after operation were compared. Efficacy was evaluated by using the Harris hip scores.Results One hundred and twenty-two hips of 104 patients were operated successfully. There were no complications of infection, fracture, neurovascular injury, surgical incisions were primary healing. All patients were followed up for a mean of 21 months (15-49 months). Postoperative review of X-ray showed: 119 hips of 101 patients remained stable and did not deteriorate; 3 hips of 3 patients deteriorated to collapse, which needed further treatment. at the last follow up, VAS score decreased and Harris hip score increased obviously compared with the pre-operation data, the differences were statistically significant (t=17.005, 10.830, all P values<0.01). Using Harris score to evaluate the clinical efficacy: excellent 99 hips, good 14 hips, general 6 hips, poor 3 hips, the excellent rate was 92.7%(113/122).Conclusions Using nanocrystallinecollagen basal bone brace combined with allograft for the treatment of adult' early femoral head necrosis will not destroy the main blood supply of femoral head, increase mechanical support in the subchondral bone of femoral head necrosis area, promote rehabilitation of femoral head necrosis and prevent collapse of femoral head, which is worthy spreading in clinical use.
