Abstract:Objective:To explore the clinical value of complete VATS thymectomy for the treatment of early-stage thymoma in patients without myasthenia gravis(MG). Methods:A retrospective review of 30 patients who underwent thymectomy by complete VATS of early-stage thymoma without myasthenia gravis during a 6-year period was conducted. Data on patient characteristics,morbidity,recurrence,and survival were collected.Results:Of the 30 patients,12 were men and 18 were women ,with mean age of 46 years(ranged from16 to 65 years),17(56.6%)for stage Ⅰand 13(43.3%)for stage Ⅱ. All procedures finished under 3 port incision on the right anterior chest,and no case was converted to thoracotomy. There was no operative death or major peri-operative complications. The time of operations was 40~200(91.7±38.1)min. The blood loss intraoperative was 30~300(106±75)ml. The hospital stay was 5~7(5.1±0.8)days. Pathological examinations after operation revealed that there were 14 cases of type A,10 cases of type AB,5 cases of type B1,1 case of type B2.All stage Ⅱcases received adjuvant radiotherapy. All patients were alive and no recurrences during the median follow-up 44 months. The estimated overall survival was (70.2±1.7)months(95%CI:66.8~73.6).Conclusions:Complete video-assisted thoracoscopic thymectomy for early-stage thymoma in patients without myasthenia gravis is a safe and effective procedure, and it is less invasive for the patient. This surgical method can be generalized in the clinic.
