Abstract:Objective To observe the curative effect of transscleral cyclophotocoagulation (TSCPC) for refractory glaucoma.Methods From November 2011 to January 2013, a prospective study was conducted on 117 patients(118 eyes) who were treated with TSCPC in the Department of Ophthalmology, Institute of Surgery Research, Daping Hospital, the Third Military Medical University (aged from 31-79 ) ; The patients were divided into group A(VA≤CF), group B(CF<VA<0.1) and group C(VA≥0.1). In three groups, the intraocular pressure was measured preoperatively and in the 1st day, 3rd day,1st month,3rd month and the 1st year postoperatively, VAs were tested preopertively and in the 1st year postoperatively; In group C visual fields(VF) were measured preoperatively and in the 1st year postoperatively. The patients who were lost to follow-up postoperatively exited statistics, and they were involved in the evaluation of therapeutic efficiency.Results Forty-four patients(44 eyes), 32 patients (32 eyes) and 28 patients(28 eyes) were successfully followed up separately in group A, B and C. The postoperative IOP of the 1st day, 3rd day, 1st month, 3rd month and the 1st year was compared with preoperative IOP in three groups separately, the differences were statistically significant(P<0.05). Preoperative VAs in three groups compared with the postoperative in the 1st year, the difference was not statistically significant(Hc=0.64, P>0.05). Preoperative MD(-17.36±4.42) of visual fields(VF) in group C compared with the postoperative MD(-16.02±4.72) of VF in the 1st year, the difference was not statistically significant(Z=-1.68,P>0.05). Complications that included 7 chemosis, 59 iridocyclitis and 5 hyphema disappeared after treatment. There were 5 eyeballs atrophied by follow-up 1year.Coclusions As an effective therapeutic method for refractory glaucoma, TSCPC can effectively maintain residual visual function of refractory glaucoma patients with different VAs. It is very safe to reduce intraocular pressure and protect optic nerve.
