Progress on the diagnosis and treatment of open calcaneal fractures

Abstract:Objective To discuss the progress on the diagnosis and treatment of open calcaneal fractures.Methods Literatures about the diagnosis and treatment of open calcaneal fractures published from January 1998 to December 2014 in PubMed, Embase and Wanfang Database of China were searched, and the languages were limited to Chinese and English. 91 articles were selected.Results The diagnosis of open calcaneal fracture should think about many-sided including the mechanism of injury, the size and depth and pollution degree of the wound, fracture classifications and compound injury. Open calcaneal fractures should be debrided thoroughly. Then the less invasive stabilization methods were applied according to the conditions of the soft tissue and the pattern of calcaneal fractures displacement. According to the different sizes and locations, the wound should be treated by delayed closure, vacuum sealing drainage drainage, free skin grafting or skin flap transposition in stage Ⅱ. For the calcaneal fractures, reduction and internal fixation through the medial wound can be done in stageⅠor open reduction and internal fixation through the lateral incision in stage Ⅱ.Conclusions The diagnosis of open calcaneal fracture includes the conditions of soft tissue and fracture classifications. To obtain the satisfying results in treatment of open calcaneal fractures, we should not only reduce the calcaneal fracture displacement, but also pay attention to the soft tissue injury, and secure the soft tissue coverage.
