Abstract:Objective To report the surgery on a rare clinical patient of intra- and extra-spinal canal tophus of cervical and thoracic and lumbar with incomplete paralysis so as to summarize the characteristics of diagnosis and treatment of the disease.Methods Data of a 50-year-old male patient with intra- and extra-spinal canal tophus of cervical and thoracic and lumbar with incomplete paralysis in September 2016 in Department of Orthopedics of Laian Jianing Hospital was retrospectively analyzed. This patient underwent decompressive right laminectomy of L4-S1 and focus debridement. During the follow-up, the patient received comprehensive treatment of low-purine diet and uric acid for the observation of the clinical outcomes; and finally, a comprehensive review of the relevant literature was conducted to summarize and analyze the diagnosis and treatment experience of spine gout.Results The patient underwent a successful procedure, and postoperative pathology confirmed that the resected mass was tophus. The postoperative symptoms were relieved, and 3 months after surgery, the strength level of grip and quadriceps was 4 degree, as well as the strength of gastrocnemius and extensor hallucis longus improved to 4 level. Six months after operation, the strength of double lower limbs restored to normal, and anesthesia relieved in the lower extremity calf lateral and dorsum and planta pedis. There was no significant impact on his daily life.Conclusions The clinical manifestations of spine gouty are lack of specificity, with difficult diagnosis, thereby pathological examination being the gold standard for its diagnosis; if patients suffer from neurological dysfunction and failed conservative treatment, the surgery should be performed as soon as possible, with postoperative diet control and drug treatment, which can achieve an satisfactory clinical outcome.
