Abstract:Objective This study aims to summarize the clinical application progress of Hounsfield unit (HU) value from CT examination to evaluate bone mineral density in spinal surgery. Methods A computer-based online search of PubMed was undertaken to identify articles about the application progress of vertebral HU values published in English from January 1980 to May 2022, with the key words of Hounsfield Unit, CT, and vertebral. A computer-based online search of the China National Knowledge Infrastructure (CNKI) database was also conducted to identify relevant articles published in Chinese from January 1980 to May 2022, with the key words of "HU值、椎体CT值" . More than 680 retrieved articles were screened, and 23 recently published papers in authoritative journals were selected for analysis, focusing on the diagnosis and treatment of spinal surgery diseases and the prevention of postoperative complications. Results The accuracy and reliability of the vertebral cancellous bone HU value in reflecting bone mineral density were better than those of the traditional dual-energy X-ray absorptiometry and quantitative CT (QCT). The vertebral HU value had an important role in predicting the risk of fusion cage settlement after lumbar fusion, screw loosening after internal fixation, and adjacent segment degeneration after scoliosis correction. The vertebral HU value also described the characteristics of the distribution of bone mineral density of the cervical spine and lumbar spine. This parameters has certain guiding significance for the formulation of spinal surgery plan and the prevention of postoperative complications. Conclusion The HU value of vertebral cancellous bone plays an important role in the evaluation of bone mineral density.
