Abstract:Objective To compare clinical outcomes of new designed elastic syndesmosis hook plate (ESHP) and ordinary syndesmosis hook(OSH)in the treatment of Syndesmosis injury.Methods Fifty one patients with Syndesmosis injury treated in the Orthopedic Department of the 175th Hospital of PLA from February 2011 to February 2012, were divided into two groups according to odd number or even number of their hospitalization date. Patients with odd number hospitalization date were treated with elastic syndesmosis hook plate(ESHP, n=27). Patients with even number hospitalization date were treated with ordinary syndesmosis hook(OSH, n=24). The operation time, blood loss during operation, incision length, and hospital stay were compared. Meanwhile, Baird-Jackson scoring system was used to evaluate the treatment effect at 1 year post-operation to observe and analyze the good rate.Results The 51 patients were successfully completed surgery. The blood loss during operation of ESHP group(22.5±5.6)mL was less than OSH group(28.5±6.0)mL. There was significantly statistical difference between two groups (t=-5.593, P<0.01). There was no statistical difference in operation time, incision length and hospital stay between the two groups(t= -1.571, 0.539, -0.476, all P values>0.05). All patients were followed up for 12-36 months, an average of 26.5 months. During the follow-up period, all fractures were primary healing. No complications such as infection, bone nonunion, malunion, breaking of internal fixator were found. According to Baird-Jackson scoring system 1 year after the operation, in elastic syndesmosis hook group, the results were excellent in 23 and good in 4, the excellent and good rate reached 100%(27/27). In ordinary syndesmosis hook group, the results were excellent in 13, good in 10 and poor in 1, the excellent and good rate reached 96%(23/24).there were significant differences between the 2 groups in the excellent and good rate (Z=2.429, P<0.05).Conclusions Compared with OSH, ESHP is more in line with the physiological micromotion characteristics of distal tibiofibular syndesmosis, which has obvious advantages in the promotion of ligament repair and tibiofibular syndesmosis functional recovery.
