Abstract:Objective To study clinical application value of high-resolution black-blood double inversion-recovery(HR-DIR-MRI) and 3D sampling perfection with application optimized contrasts using different flip angle evolutions(3D-SPACE) on demonstrating the distal dural ring (DDR) and the fine anatomical structure of the carotid cave. Methods A prospective study were performed for 20 healthy volunteers recruited from April to October 2015, including 11 males and 9 females, aged from 19 to 35 years old, with an average of 27 years old. The 3D-SPACE and HR-DIR-MRI techniques were used to scan the DDR and carotid cave, the morphology and walking of DDR and the morphology of the carotid artery were observed, besides, the depth of the carotid cave by 3D-SPACE-T2WI and HR-DIR-H2WI images was measured. DDR side of the two sides of the plane inclination angle, front and back side of the plane tilt angle were measured and compared by HR-DIR-T2WI image. The signal to noise ratio (SNR), contrast to noise ratio (CNR), and DDR image quality score on 3D-SPACE were compared with HR-DIR-MRI images. Results In 3D-SPACE-T2WI and HR-DIR-T2WI sequences, coronary DDR 40 side slopes slightly downward in the lateral-to-medial direction, sagittal showed DDR 36 side slope slightly downward in the anterior-to-posterior direction, 4 side slope slightly downward in the posterior-to-anterior direction. There were no significant differences on 3D-SPACE-T2WI, HR-DIR-T2WI DDR lateral-to-medial angle and the anterior-to-posterior angle between different sequences (all P values>0.05). HR-DIR-T2WI sequences of coronal and sagittal DDR imaging SNR, CNR and image quality score were higher than in 3D-SPACE-T2WI sequence, the difference had statistical significance (P<0.05). On the 3D-SPACE-T2WI sequence, the 23 sides of the carotid cave were observed, including type B 16, type C 7 and type A 17. Depth of carotid cave about 1.0-7.5 mm, average (2.07±1.31 )mm. On the HR-DIR-T2WI sequences, the 19 sides of the carotid cave observed, including type B 13, type C 6, and type A 21; carotid cave depth of 1.1-7.5 mm, average (2.13±1.33)mm. There was no significant difference in the depth of carotid cave between the two sequences(P>0.05). Conclusions Combining with high resolution MRI multi sequence technology, DDR can be located more accurately, and then identify the aneurysm. It has good clinical value to assess the condition and choice of treatment methods. In MR 3D-SPACE based on the application of HR-DIR-MRI sequence can clearly display the DDR and the anatomy of the carotid cave around fine. HR-DIR-MRI optimizes the DDR image by using double inverted black blood technique and pulse gating technology.
