Abstract:Objective To investigate and observe the short-term clinical and radiographic outcomes of total hip arthroplasty with application of modular porous tantalum cups with morselized or structural bone grafting for lacunar acetabular defects.Methods Clinical data of patients operated in Union Hospital, Huazhong University of Science and Technology from November 2011 to May 2013 were retrospectively analyzed. The Results of 43 patients (49 hips) with simple lacunar or combined segmental acetabular defects receiving total hip arthroplasty (37 patients, 42 hips) or total hip revision (6 patients, 7 hips) using a modular porous tantalum cup and intra-operative bone grafting were involved. There were 20 males (23 hips) and 23 females (26 hips), aging from to 48 to 71, (average, 56±7 years). Operation time, incision length, blood loss and postoperative complications were recorded routinely. The clinical outcomes were evaluated by Harris hip score, visual analog scale (VAS) and Western Ontario and McMaster Universities Arthritis Index (WOMAC). The radiographic illustrations were assessed by DeLee-Chanley measuring system and Engh's imaging evaluation.Results Average operation time was (87±18)min(ranged from 70 to 110 min), average blood loss was (850±200) mL(ranged from 600-1 200 mL), and incision length was (12±2)cm(ranged from 10-15 cm). Average follow-up time was 37 months (ranged from 18 to 45 months).After three months,one year postoperation and last follow up, the Harris hip score was increased, VAS and WOMAC score were decreased obviously compared with pre-operation data, the differences were statistically significant(all P values<0.05). Harris score was used to evaluate the hip function 6 months after operation: excellent 30 hips, good 19 hips, the excellent rate was 100%. Twelve months after operation, all patients had close contact with the acetabular cup and bone surface, bone defect filled with good and no prosthesis loosening.Conclusions The relative short-term follow-up of our patients indicates that total hip arthroplasty or total hip revision using modular porous tantalum cups combined with bone grafting can achieve favorable early clinical and radiographic outcomes while long-term effects still needs further investigations.
