Sternal variations observed by multimodal multislice spiral CT

Abstract:Objective To investigate the sternal variation observed by multimodal multislice spiral CT (MSCT). Methods The imaging data of 1 000 patients who underwent chest MSCT examination in Tai'an Central Hospital from January to June 2020, a total of 505 males and 495 females aged 10-99 (54.7 ± 25.5) years were retrospectively studied. All chest CT scans were performed with transverse volume scan, where multiplane reconstruction, maximum density projection, and volume rendering imaging of the sternum were performed using bone algorithm to show the full view of the sternum. The incidence of sternal manubrium variation, sternal body variation, xiphoid process variation and sternal fusion were counted. Results (1) Manubrium sternum variation: 56 cases (5.6%) were suprasternal, 27 cases were unilateral, and 29 cases were bilateral. Suprasternal nodules were found in 63 cases (6.3%), namely, unilateral in 31 cases and bilateral in 32 cases. Suprasternal protrusion was found in 12 cases (1.2%). (2) Sternal body variation: 62 cases (6.2%) were "O" shaped sternal body. Foramen sternum in 40 cases (4.0%) and substernal fissure in 5 cases (0.5%) were observed. (3) Variation of xiphoid process: 347 cases (34.7%) with terminal variation of xiphoid process, 46 cases with deletion of xiphoid process, 286 cases with double tail of xiphoid process, and 15 cases with three tail of xiphoid process were recorded. Two hundred three cases (20.3%) with internal variation of xiphoid process, 118 cases with single xiphoid process, five cases with double xiphoid process, and 80 cases with false xiphoid process were also reported. Xiphoid process morphologic variation in 61 cases (6.1%), deflection in 35 cases, and elongation and distal warping in 26 cases were also observed. (4) Sternal fusion variation: 192 cases (19.2%) were pedicle fusion, 148 cases complete fusion, and 44 cases partial fusion. A total of 509 cases (50.9%) with body-sword fusion, 276 with complete fusion, and 233 with partial fusion were also observed. The first and second segments of the sternum were not fused in 30 cases (3.0%) and partial fusion in 39 cases (3.9%). The total sternum fusion was observed in 60 cases (6.0%). Conclusion Sternal variation is common and multimodal MSCT can be used to observe sternal variation.
