Abstract:Objective This study aimed to evaluate the feasibility and efficiency of surgical pulmonary embolectomy for acute pulmonary embolism. Methods The retrospective study was conducted. Clinical data of 51 patients with acute pulmonary embolism who received surgical pulmonary embolectomy in the First Affiliated Hospital of Anhui Medical University from October 2016 to July 2020 were analyzed. Of the patients, 22 were males, and 29 were females, with age of 61.7±13.1 (range, 22–78 years). During postoperative course, the clinical symptoms, occurrence of postoperative complications, pulmonary artery systolic pressure, and degree of tricuspid regurgitation were improved. Results All patients were successfully operated except for one patient who died of massive intraoperative respiratory tract hemorrhage. The median operative and cardiopulmonary bypass time were 221 (interquartile range, 139–303) min and 81 (interquartile range, 44–118) min, respectively. Clinical symptoms significantly relieved in 50 patients within 24 h after surgery. In addition, the changes in vital sings such as respiratory rate, heart rate, oxygen saturation, and arterial partial pressure of oxygen were significantly improved, and the difference was statistically significant (all P values <0.05). No death, recurrence of pulmonary embolism, or adverse bleeding events were recorded during follow-up from 7–41 months (average 19 months). Based on the echocardiography results of 34 patients at 1 and 6 months after operation, the pulmonary artery systolic pressure and degree of tricuspid regurgitation were significantly improved compared with that before operation (all P values <0.05). Conclusions Surgical pulmonary embolectomy is an effective treatment, which can be performed with satisfactory improvement of heart function for patients with acute pulmonary embolism, with a low rate of postoperative mortality and complication.
