The progress on repairing of finger injuries associated with the segment defect of the proper digital artery

Abstract:Objective To summarize the current status and treatment progress of the hand defect with the proper digital artery segment defect.Methods Using “指动脉缺损”“皮肤软组织缺损” and “digital artery defect” “soft tissue defect” as key words in the CNKI Data, Wanfang Data, EMbase and PubMed Data from January 2000 to December 2018. Six hundred and thirty-two references were retrieved, and unavailable to obtain full texts, repetitive studies, and low quality literatures were excluded, consequently 51 references were included. Then to analyze and summarize the advantages and disadvantages of those papers.Results With the consecutive development of microsurgical technology, the popular of the perforator flaps, the elevated requirements of doctor and patient on the results of repair and the continuous improvement of the repair concept, the goal of the treatment of the hand defect with the proper digital artery segment defect develops to less damage, and good efficacy, simultaneously. And the methods have gradually developed from the conventional combining vessel graft with flaps, the adjacent homodigital artery island flaps to the flow-through flaps, which could achieve the result of one stage to repair the main artery, and meanwhile to cover the tissue defect.Conclusions The flow-through flap is the optimal treatment for the finger defect with the proper digital artery segment defect, including the advantages of the perforator flap and flow-through bridging the proper digital artery, which can effectively solve the main vascular injury reconstruction and wound coverage by only one stage. Additionally, it abides the current concept of repair and reconstruction, and has the greatest economic efficiency.
